Liability/Photo & Video Release/Marketing Communication

Liability Release:

I undersigned, being at least 18 years of age, in exchange for the event venue, LIVE LOVE SPA & ALL PARTICIPATING VENDOR SPONSORS allowing me to utilize the facilities, agree that the event venue, LIVE LOVE SPA & ALL PARTICIPATING VENDOR SPONSORS and affiliates/agents/employees are released from all liability for any type of injury or damage that may result from my use of the facilities, regardless of this results from my own negligence, the negligence of a third party or the negligence of the event venue, LIVE LOVE SPA or any PARTICIPATING VENDOR SPONSORS their employees/affiliates/agents.

I assume ALL risks of accidental injury to myself that may occur during the use of facilities whether it is the result of my own negligence, the negligence of a third party or the negligence of the event venue, LIVE LOVE SPA or any PARTICIPATING VENDOR SPONSORS their employees/affiliates/agents.

I understand and agree that this is a release and assumption of risk agreement.

Please advise us of any health conditions upon registration including:

Heart Disease, Pregnancy, Diabetes, Varicose Veins, Medications, Skin Conditions, Recent Surgery, Chemotherapy, High Blood pressure

Assumption of the Risk and Waiver of Liability Relating to

You further acknowledge and agree to accept sole responsibility for any injury (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, to you or those with whom you come into close contact (including your household members)(“Claims”). You hereby release, covenant not to sue, discharge, and hold harmless Live Love Spa, its employees, board of directors, and representatives, of and from any Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.

You further understand and agree that this release includes any Claims based on actions, omissions, or negligence of Live Love Spa, its employees, board of directors, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Live Love Spa event.

You agree to abide by posted rules, warning or instructions by Live Love Spa, the event venue where the event is held and any government authority with respect to social distances, adherence to any dress code (including mandatory face coverings) and sanitation guidelines.

You further agree not to attend the event if you are diagnosed with or suspect that you may have been infected by COVID-19, and in the event you become ill or suspect you have been infected with COVID-19, you agree to immediately discontinue your participation in the event and alert Live Love Spa via e-mail or telephone.

If you cannot agree to or accept the foregoing, DO NOT ATTEND THE LIVE LOVE SPA EVENT. Failure to agree to or abide by any of the foregoing may result in denial of entry or immediate expulsion from the event, in Live Love Spa’s sole discretion, in addition any and all remedies which may be available to Live Love Spa, in equity or at law.

LIVE LOVE SPA may have photographers and videographers present during events. If so, we ask that you kindly review & agree to our our photo/video release.

Without remuneration of any kind, I being competent and of legal age, hereby give LIVE LOVE SPA the absolute and irrevocable right and permission, with respect to my likeness, performance, and participation in its Video/Web Banners and Photography communications;

a) To record/photograph my likeness, performance and participation;

b) To copyright the same in its own name or in any other name which it may choose;

c) To telecast the Communications of the recording thereof one or more times over any Internet site, station or stations, or to publicize the Communications or any portion thereof by any means, for any purpose whatsoever in whole or in part, including (but not by way of limitation), promotion, advertising, trade; and

d) To use my name in connection therewith if it so chooses.

I acknowledge that LIVE LOVE SPA, are and will be the sole owner of all rights in and to the Communications and the recording/photography thereof, for all purposes in perpetuity. I hereby assign any copyright rights, publicity rights or any other rights that I may have regarding the Communications to LIVE LOVE SPA I also hereby release LIVE LOVE SPA from any and all claims of any nature whatsoever which I could or might have against the Releases by reason of any fact or matter whatsoever.

By proceeding, I acknowledge that I have carefully read, understand & agree to the photo/video release.
